STAY WELL PROGRAM

Our goal through the Stay Well Program is to assist Hunger Center clients to increase and maintain wellness for themselves and their families with the minimal resources available to them.


Overview

When you can’t find or afford nutritious food, everything in your life suffers, including your health. The Hunger Network’s Stay Well Program is a community outreach program that focuses on health education and promotion in efforts to bridge the gap between hunger and health. Diseases including obesity, diabetes and heart conditions are often developed or made worse by poor diet and unhealthy lifestyle choices. The goal of the Stay Well Program is to improve the opportunities for food insecure clients to access health care and prevention strategies, through direct service out in the community and targeted health education.

 

            

What We Do

In partnership with University Hospitals’ Department of Family Medicine & Community Health, The Stay Well Program provides monthly community health events at a number of our Hunger Centers throughout Cuyahoga County.
Our events include:
-Preventative health screenings [blood pressure, blood sugar, cholesterol, BMI, body fat %, height, weight, diabetes & stroke risk assessments]
-Medical navigation through Carmella Rose Health Foundation 
-Wellness counseling [information on diet & nutrition, physical activity, disease prevention, smoking cessation, etc.]
-Healthy food demonstrations, samples & recipes
-Educational information/presentation on monthly health topic [i.e. cancer awareness, ins and outs of taking your medications, diabetes, cold & flu, dental health, hearing & vision, mental wellness]

OPPORTUNITIES OF INVOLVEMENT

Help connect neighbors in need by supporting the Hunger Network’s Stay Well Program in one of the ways:

Volunteer: professional volunteer or medical students to help administer health screenings and event coordination.
In-kind Donations: donations serve as incentives and rewards for clients attending monthly health discussions.
Monetary Donations: funds are used to purchase materials that support monthly discussion topics.
